Form 5B Certificate
The official certificate prescribed in the Central Motor Vehicles Rules 1989. Issued only by accredited ADTCs — it is your legally recognized proof of driving competence and your direct route to a driving licence without the RTO test.
What is Form 5B?
Form 5B is the official certificate prescribed under the Central Motor Vehicles Rules 1989. It is the formal document that confirms a candidate has successfully completed the training course at an accredited ADTC and has demonstrated the required driving competence through objective, automated evaluation.
Critically — under CMVR Rule 15 — the holder of a valid Form 5B is legally exempt from the standard RTO driving test. Submit it with your other documents to the RTO, and your licence is issued without any further driving test.

How to Get Your Form 5B
Form 5B can only be issued by an officially accredited ADTC. You cannot purchase it, request it separately, or obtain it without completing the full training and evaluation programme.
Enrol at ADTC Firozabad
Complete registration with documents and pay the course fee.
Complete Theory + Practical Training
Attend all classroom sessions and complete all driving practice hours.
Pass Automated Evaluation
Clear the objective assessment on the Automated Driving Test Track.
Receive Form 5B
ADTC issues your Form 5B certificate. It is valid immediately upon issue.
Submit to RTO — Get Licence
Take Form 5B + other documents to the RTO. Licence issued without a driving test.
Submitting to the RTO
Once you hold Form 5B, visit your local RTO (Regional Transport Office) with the following set of documents to apply for your Permanent Driving Licence:
- Form 5B (original, issued by ADTC)
- Licence application form (Form 9)
- Identity proof (Aadhaar / Passport / Voter ID)
- Address proof
- Medical certificate (Form 1A)
- Passport-size photographs
- Prescribed licence fee
The RTO cannot demand an additional driving test from a valid Form 5B holder. If documents are in order, the licence is issued directly. If you face any issue, contact the UP Transport Department grievance cell.
Important Things to Know About Form 5B
1–2 Year Validity
Form 5B is valid for a period notified by the government — typically 1 to 2 years from the date of issue. Apply for your licence within this window.
Valid Across India
Prescribed under central law (CMVR 1989). Recognized by every RTO in every state — not just Uttar Pradesh.
ADTC-Exclusive
Form 5B can only be issued by an officially accredited ADTC. It cannot be obtained from any regular driving school, RTO, or any other authority.
Multiple Categories
Form 5B can be issued for two-wheelers, Light Motor Vehicles, and eligible commercial vehicle categories — each trained separately.
Form 5B is not a driving licence. It is a certificate that exempts you from the driving test so that the RTO can issue the licence more quickly. The RTO remains the sole authority for issuing driving licences in India. If your Form 5B expires before you apply, you must appear for the standard RTO driving test.
